ATTENTION PARENTS OF SIXTH GRADE STUDENTS:
Please be aware that all students entering the 7th grade are required by the State of Florida to have had a Tdap immunization prior to entering school. A student who does not have the required proof will be excluded from school until documentation is provided. Immunizations can be obtained from your own doctor or through the Health Department and must be on a DH 680 blue form. If you have any questions, please do not hesitate to call the school.
